AUTHORITY: DAF is authorized to collect the information requested on this form pursuant to 10 U.S.C. 9013, Secretary of the Air Force, as implemented by Air Force Instruction 36-2608 Military Personnel Records System, AFI 36-2110 Total Force Assignments, AFI 36-2406 Officer and Enlisted Evaluation Systems, AFI 36-2670 Total Force Development and AFMAN 36-2664 Personnel Assessment Program. DAF is authorized to collect your Social Security Number (SSN) by Executive Order 9397 (November 22, 1943), as amended by Executive Order 13478 (November 18, 2008).
PURPOSE: The information is collected to ensure accurate and positive identification of the individual for the maintenance of military personnel records and to facilitate execution of Senior Leader Career Management operations. SSN may be used as individual identifiers for actions/processes related to senior leader progression paths, development, PME, assignment/retirement actions, board related actions, special programs, assessments, biographies, official photographs, and performance/evaluation actions, procurement, education and training, classification, assignment, career development, evaluation, promotion, compensation, sustentation, separation and retirement.
ROUTINE USES: In addition to those disclosures generally permitted under 5 U.S.C. 552a(b) of the Privacy Act of 1974, as amended. The information you provide may also be disclosed externally to other Federal agencies or Congressional offices which may have a need to know it in connection with your performance of duties and professional development.
